Job Description:
Provide skilled occupational therapy services to patients in their homes under physician orders
and the established plan of care. Perform comprehensive occupational therapy evaluations and
ongoing reassessments. Develop and implement individualized treatment plans to improve self-
care, mobility, and functional performance. Assess home environments and recommend adaptive
equipment and safety modifications. Provide patient and caregiver education on compensatory
strategies, energy conservation, and safety. Collaborate with nursing, therapy, and provider
teams to support coordinated patient care. Complete accurate, timely, and compliant clinical
documentation.
Requires a U.S. Master’s degree or foreign equivalent degree in Occupational Therapy. Requires
a current, active Occupational Therapist license in the State of Colorado. Requires one (1) year
of experience as an Occupational Therapist. Must have the authority to work permanently in the
U.S.
